There are actually a couple of different styles. This one is my new style, Dark v2.0

The others, Dark (v1) and Default are selectable by going to the bottom left corner of the page and opening the drop down window.

Ahh, that's because there was a bug in the application form. When I was getting them for a while, I'd get the questions and no answers, even though I know you guys filled out every response (I made it so that you couldn't submit the form otherwise).

Go ahead and try again if you haven't already. I fixed the bug.
